January 1, 2009 – Department of Labor/OSHA changed OSHA Outreach 10 & 30 Hour training and reporting requirements. All requests must use the 2009 forms.

- Design, develop and deliver your training
- OSHA 10 Hour Construction Industry Presentations and Lesson Plans
- OSHA 10 Hour General Industry Presentations and Lesson Plans
- Complete the 2009 Outreach Program Report (see download training report below)
- Statement of Certification MUST be signed and dated.
- Include trainers name, trainer course and expiration date.
*If this is your first request or have changed addresses include mailing address, phone and email.
- Identify how many hours were spent on each topic taught.
* Breaks and lunches do not count as class time.
- 10 HOUR COURSES MUST TOTAL 10 HOURS OF TRAINING
- 30 HOUR COURSES MUST TOTAL 30 HOURS OF TRAINING
◦Include roster of attendees *DO NOT SEND DAILY SIGN IN SHEETS
- must be legible – if cannot read cannot issue cards
- request with 50 or more students will not be processed unless trainer received prior approval (contact outreach coordinator for approval)

- Submit your completed outreach program report and a copy of your trainer card or certificate to the Rocky Mountain Education Center (RMEC)
- Each Outreach Training Request must be submitted separately.
- Each training request must have an original end date of training.
- All training must be submitted within 6 months of end date.

- Trainers who have previously received outreach cards from the RMEC and have been issued and RMEC trainer number can submit training using the online link below:
Request that do not include all of the above information and/or do not meet the minimum training requirements will not be processed and issued cards.
Submission will be reviewed for:
- Trainer qualification (valid authorization status)
- Compliance to the guidelines (topics, time, etc.)
- Completeness of information
Correct, complete and valid requests will be processed within 4-6 weeks of receipt.
Requests will be accepted via, email, fax or mail.
Trainers must retain outreach class files for five years. The file for each class must include:
- Student sign-in sheets for each class day
- Student addresses
- Copy of the documentation sent in to request cards, including a list of the topics taught and the amount of time spent on each
- Records which indicate the card number dispensed to each student. One way to accomplish this is to make a copy of all the cards you issue.
OSHA reserves the right to request copies of class records for verification purposes.

Outreach Cards – if a participant looses a 10 or 30 hour card, Outreach Trainers will need to:
- Verify attendance of the participant on an outreach program report form
- Resend the original request with an explanation of the reason for the replacement
- 10 & 30 hour outreach cards cannot be replaced if the original training date was more than 5 years ago. Participants will have to attend a new 10 or 30 hour training to be issued a new card.
