Replacement Card/Certificate Request
This form is for students that have attended a course put on by the Rocky Mountain Education Center and need a replacement certificate and card.
**This is not for 10 or 30 hour replacement cards, please contact Jeannine Kreller at 888-329-7076 or email at outreach@rrcc.edu**
Instructions:
- Before you fill out this form. You will need to know the location of where you took the course and the dates.
- Fill in your full name as it appeared on your previous certificate, address, city, state, zip and phone number.
- Fill in the name of the course, the date and location.
- We do require a $25 replacement fee, you can either submit this form online and call in with a credit card number or please print out the form and send a check to:
